What documents should I bring with me for my first appointment?
A Medicare card (and/or any government affiliated dental coverage info.), up-to-date dental insurance coverage info. (if any), current list of medications (if any), new patient dental form completed (or fill out form at our office). Also, please notify reception of any recent X-rays/Panorex (within last 2-3 years), if you wish to obtain these for your file.
When should I arrive for my appointment?
For new patients, please arrive 5-10 minutes beforehand to fill it out the health form (if you have not already done so). For returning patients, it is sufficient to arrive 5 minutes beforehand or at the time of your appointment.
How do I reschedule or cancel my appointment?
Call us at (514) 695-8010. Please make sure to do so more than 24 hours before your appointment time. If you cancel or reschedule less than 24 hours in advance, we’ll have to charge you a $50/hour fee. This is because we reserved that time specifically for you.
Do you take dental insurance?
We work with a variety of dental insurances and are happy to submit insurance claims for reimbursement or estimates of coverage electronically on your behalf. Feel free to give us a call, our reception can help outline your insurance plan benefits.
